PRJ 2261 - Neighbourhood Health Community Prevention and Empowerment

This service is an integrated, targeted healthy lifestyle behaviour service for high-risk communities in North Croydon, covering the North East and North West Croydon localities. It is designed to reduce preventable illness, premature mortality and health inequalities by supporting residents most at risk of poor health outcomes to make sustained changes in relation to healthy weight, smoking cessation and alcohol reduction. The service focuses on communities experiencing higher levels of deprivation, housing pressure, unemployment, limited access to healthy assets and increased cardiometabolic risk. The service will provide structured, evidence-based interventions culturally tailored engagement, community outreach, accessible delivery locations, and digital options where appropriate. It will complement rather than duplicate existing Croydon Council universal healthy behaviour services, with clear referral and signposting arrangements between services. The three main service components are: Weight Management For Targeted High-Risk Residents Providers will be required to deliver targeted outreach in North Croydon aimed at people living in areas of socioeconomic deprivation (IMD quintiles 1-2), those form Black African and Black Caribbean communities, those from South Asian communities; and those with SMI or LD living in the North Croydon. The service will deliver culturally sensitive structured, evidence-based weight management support to targeted residents who meet the service specification defined high-risk target eligibility criteria to achieve clinically meaningful weight loss and sustain a healthy weight. Smoking Cessation For Targeted High -Risk Residents Providers will be required to target residents living in deprived neighbourhoods IMD quintiles 1-2 of North Croydon where smoking prevalence and cardiovascular disease burden are highest. Engagement should focus particularly on communities with elevated cardiovascular risk, including Black African and Black Caribbean populations, those from South Asian communities; and Eastern European residents alongside men aged 40-65 who are more likely to smoke/drink and develop hypertension and/or diabetes. Recruitment should also leverage existing NHS pathways such as GP hypertension or diabetes registers to identify smokers at highest risk of cardiovascular events. The service will deliver culturally sensitive structured smoking cessation support to those who meet the service specification defined high-risk target eligibility criteria. The objective is to increase treatment uptake, support quit attempts, and achieve validated abstinence outcomes, thereby contributing to a sustained reduction in smoking prevalence and smoking-related harm across the locality. Alcohol Reduction For Targeted High -Risk Residents: Providers will be required to target residents living in deprived neighbourhoods IMD quintiles 1-2 of North Croydon. Engagement should focus particularly on communities with elevated cardiovascular risk, including Black African and Black Caribbean populations, those from South Asian communities; and Eastern European residents alongside men aged 40-65 who are more likely to smoke/drink and develop hypertension and/or diabetes. The service will deliver culturally sensitive structured support to those who meet the service specification defined high-risk target eligibility criteria. Outcomes will be focused on brief interventions, extended support, reduced AUDIT-C scores and signposting to specialist services where dependence is indicated. The provider will be expected to establish referral pathways from GP practices, Croydon Council services, VCSE organisations and self-referrals, subject to validated eligibility evidence. The provider must also work closely with local partners, including primary care, Croydon Council, voluntary and community organisations, adult healthy behaviour services, smoking cessation services, weight management services, substance misuse providers and wider social support services. The service will be delivered under a block contract for the period 1 April 2027 to 31 March 2030, with an option to extend by one year. The annual contract value is £184,539 (Inclusive of VAT) and the total contract value is £738,156 (Inclusive of VAT), with indicative component allocations for smoking cessation, alcohol reduction and weight management. Providers may propose collaborative or consortium delivery models, provided there is a single lead contractual entity accountable for performance, governance, reporting and compliance. Performance will be monitored through quarterly reporting on key performance indicators specified in the service specification.The service aligns with NHS prevention priorities, the NHS Long Term Plan, Core20PLUS5, NICE guidance and local Croydon population health priorities.

The SWL Central Croydon Prevention, Access and Health Equity Service is a targeted prevention service for Central East and Central West Croydon, running from 1 April 2027 to 31 March 2030, with an option to extend by one year. The service is designed to address health inequalities by focusing on population groups whose health outcomes are affected by digital exclusion in Central East Croydon and anxiety related to financial and housing insecurity, or lower childhood immunisation uptake in Central West Croydon. The service has three main components: Central East Croydon Digital Inclusion and NHS App Uptake Programme This element supports residents aged 40 and over who are registered with a Central East Croydon GP practice, experience barriers to digital access, and are not currently registered with the NHS App. The programme will provide outreach, one-to-one support practical guidance to enable residents to register and use the NHS App and access online NHS services more confidently. Central West Croydon Welfare, Financial and Housing Advice Service This element targets adults in deprived areas of Central West Croydon who are experiencing financial, housing or welfare-related stress, have moderate to high anxiety, and either have a long-term condition or high primary care usage. The service will provide structured casework which has the potential to cover debt and benefits advice, housing advocacy and financial literacy support, with the aim of reducing anxiety, improving wellbeing and addressing wider determinants of poor health. Central West Croydon Childhood Immunisation Equity Programme This element focuses on improving MMR vaccination uptake among Black and minority ethnic families with children aged 0-5 who are registered with Central West Croydon GP practices and have not yet had MMR vaccination. It will use culturally competent outreach, two-way engagement, navigation support and follow-up to help families access routine immunisations. The service will be delivered through targeted outreach, community-based engagement, partnership working and flexible delivery models. Providers will be expected to work with GP practices, VCSE organisations, local authority services and other relevant partners to avoid duplication and ensure residents receive joined-up support. Performance will be monitored through quarterly reporting on key performance indicators specified in the service specification. The annual contract value is £209,063 (Inclusive of VAT) and the total contract value is £836,252 (Inclusive of VAT). Providers may bid as a single organisation or through partnership, consortium or lead-provider arrangements, with one lead contractual entity accountable for performance and governance.

The SWL South Croydon Weight Management, Independence & Wellbeing Service is a targeted prevention and empowerment service covering South East and South West Croydon. It is designed to address different local health needs through three linked service components: targeted weight management support, carer strain support, and loneliness/social connection support. In South East Croydon, the service will provide targeted weight management interventions for adults aged 25-64 who are at higher risk of poor health outcomes, including people living in more deprived areas, residents from higher-risk ethnic groups, and people with severe mental illness or learning disabilities. The aim is to support those who meet the service specification defined high-risk target eligibility criteria to deliver sustainable behaviour change, clinically meaningful weight loss, and appropriate onward referral into wider lifestyle and long-term condition support. In South West Croydon, the service will focus on supporting an older population and those affected by long-term conditions. One pathway will support a target group as defined in the service specification of unpaid carers experiencing high levels of carer strain, helping them build coping strategies, access practical and emotional support, and reduce the risk of burnout. A second pathway will support a target group as defined in the service specification of older residents experiencing loneliness and social isolation through social prescribing, group activities, befriending and personalised medium-term support planning. The service will be delivered through targeted outreach, accessible community-based provision, culturally appropriate engagement, and close partnership working with primary care, Croydon Council, voluntary and community sector organisations, and existing local services. Providers will be expected to maintain robust governance, safeguarding, information governance, reporting and outcome monitoring arrangements. Performance will be monitored through quarterly reporting on key performance indicators specified in the service specification. The contract is proposed to run from 1 April 2027 to 31 March 2030, with an option to extend by one year. The annual contract value is £169,430 (Inclusive of VAT) and the total contract value is £677,720 (Inclusive of VAT)
