Provision of home and community-based care and support services under SDS Option 3 (2027 Angus Framework)

Adults aged 16 and over who have a diagnosed learning disability and who require support with a variety of needs within their home and in the community. This relates to Adult service users aged 16 – 64 who present with significant challenging behaviour and/or complex autism.
